aKinderWellness Testing and Quality analysis

The “From Where” dropdown in the Client Intake Form does not populate organization names after enabling the question “Are you already receiving assistance from other organizations?”.
Instead, it only displays a single invalid placeholder option: Select.

User Role: Intake Adminn

Steps to Reproduce

1. Login as a user with role Partner Manager
2. Open the Client Intake Form
3. Enable: Are you already receiving assistance from other organizations?
4. Open the “From Where” dropdown

Actual Result
The dropdown displays only:Select
No organization names are shown.

Expected Result
The dropdown should display the list of organization names from where the client may already be receiving assistance.
