Description
The Intake Form progress bar does not reflect the correct completion status after submitting the form: shows 37 of 43 questions answered

Steps to Reproduce

  1. Open Intake Form
  2. Fill all required fields
  3. Submit the form
  4. Observe progress → shows incomplete (e.g., 37/43)

Actual Result
Progress remains incorrect after submission of all fields

Expected Result
Progress should correctly show 43/43
