Duplicate Questions in Client Intake Form (App)

Description

The intake form displays duplicate questions related to the same input. Specifically, the question “Why do you need us?” appears twice—once as a dropdown selection and again as a text input field labeled “Enter why do you need us?”. This creates confusion for users and leads to redundant data entry.

Environment:
Platform: Android (Web)
Browser: Chrome
Device: Mobile

Steps to Reproduce:

  1. Open the Client Intake Form.
  2. Scroll to the “Required Information” section.
  3. Observe the question “Why do you need us?” appears as a dropdown.
  4. Continue scrolling.
  5. Notice another field labeled “Enter why do you need us?”.

Expected Behavior:
Each question should appear only once

Actual Behavior:
The same question is presented twice in different formats, causing redundancy and confusion.
